Educational Psychology Service, School Referral Form
The Educational Psychology Service (EPS) referral form is designed to be used for pupils within Stage 3 or Stage 5 of the Code of Practice and completed by the school. Parents who believe that their child should be referred are advised to contact the school.
The new form should be used from 1st September 2006, however referrals on the current form will be accepted until Friday 29th September 2006. The form can be completed electronically. However, please note that, for security reasons the completed form should be signed and posted to the relevant EPS Regional Office.
Should you have any queries about the form please contact your school’s named Educational Psychologist or Senior Educational Psychologist based at the local office.

Increasingly parents are seeking private reports from educational psychologists, clinical psychologists and other educational consultants and presenting them to Boards’ Educational Psychology Services for action, usually with the purpose of securing Stage 3 provision, such as literacy support.
